Demonstrates the knowledge and skills necessary to provide service appropriate to the age of the patient. Assesses patients and identifies needs that encompass the total well-being of the patients. Develops an effective plan of treatment in collaboration with the referring, attending physician, medical director and multidisciplinary team. Instructs the patient/family about hospice services and the individualized plan of care. Observes, records and reports to the physician any reaction to treatment or changes in the client's condition. Performs supervisory visits of In-Home Aides a minimum of every two weeks. Participates in Continuous Quality Improvement activities. Assists in the education of medical students/residents, nurses, nursing assistants, volunteers and others as identified. Demonstrates professionalism. Demonstrates Service Excellence. <br />Patient Safety and Quality Improvement <br /> <br />Current NC RN licensure required. Current NC driver's license or be licensed in the state you reside and maintain the UHS required minimum insurance limits required. Two or more years of progressive clinical experience with a minimum of one year experience in hospice preferred. Regular full-time (1.0 FTE) Position is located in Ahoskie, NC
