Career Opportunity

Behavioral Health Triage (Outreach) part time 10 hrs wk

The care coordinator is responsible for the delivery of discharge planning and social work services to all patient units of the hospital and designated outpatient treatment units. It requires interaction with a variety of patients including but not limited to adult, geriatric and mentally retarded populations. The care coordinator will provide direct intervention with patients and families and jointly with the health care team, will provide assessment, problem solving, advocacy, community referrals and crisis intervention services. Care coordinators are accountable for the daily inpatient management of all discharge planning, social work and utilization review functions for the assigned patient care areas. Primary responsibilities include the proactive development, implementation and ongoing revision of an interdisciplinary post hospital plan of care.

Education/ Professional Requirements:
-Master's Level Licensed Therapist.
-Minimum provisional licensed required, full license:
LCSW/Licensed Clinical Social Worker,
LMFT/Licensed Marriage and Family Therapist, or
LPC/Licensed Professional Counselor strongly
preferred.
-Experience in working with behavioral health patients;
preferably in an inpatient facility.
-Experience in individual, group and family therapy as
well as discharge planning and case management
desired.

-Ability to work independently and take initiative
-Demonstrates accountability.
-Must be able to manage several job tasks
simultaneously and able to function effectively under
demanding situations.
-Demonstrates a positive attitude and expresses
concerns in the appropriate manner and setting.
-Demonstrates excellent human relation skills.
